& Major Responsibilities
|Is the primary case manager for an assigned number of patients. Performs a variety of therapeutic and service assignments concerned with the treatment of alcoholism and other drug dependency in the primary, family and aftercare phases of the program.
Responsible for day-to-day provision of primary counseling services in accordance with program policies and procedures.
Responsible for communication with all referents, external sources to facilitate patient care and progress in treatment.
Meets with social workers, county, probation officers, employee assistance counselors or other concerned parties as needed, with appropriate consents.
Assesses, identifies and addresses the special needs of assigned populations that may include adolescent, adult and geriatric patients when directly or indirectly providing treatment or patient care.
Keeps aware of program needs and changes, and offers input regarding changes to management in order to facilitate patient care.
Contributes to the development, implementation and evaluation of the patient’s master treatment plan in collaboration with other team members.
Seeks opportunities for professional development to ensure that knowledge and competence is current in the area of chemical dependency treatment.
Must meet one of the Minnesota DHS qualifications categories for counselor.
One to two years experience in chemical dependency treatment field.
30 hours of training in adolescent development required for Adolescent Program Staff.
Special Knowledge, Skills, and Abilities:
Thorough knowledge, experience and application of self-help models (i.e., A.A., N.A.).
Familiarity with all aspects of the continuum of care for chemical dependency.
Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all age groups (neonatal through geriatric).
Must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient’s status, and interpret the appropriate information needed to identify each patient’s needs in relation to age and to provide the care needed as described in the department’s policies and procedures.
