Primary Function & Major Responsibilities Supervise the total care management of clients, utilizing the comprehensive base of physical therapy. Performs medically prescribed Physical Therapy procedures, evaluations and diagnostic tests on clients. Also, provides advisory service to other professional workers who share the responsibility for that client's care. Maintains confidentiality in all aspects of the job. Maintains accurate client records. Instructs family, clients and/or other home health care professionals in appropriate home care treatment. Interacts appropriately with management and professional peers to ensure compliance with appropriate agency standards. Provides advisory and consultation services to staff to ensure quality care. Promotes personal safety and a safe environment for clients and co-workers. Performs other Duties/Responsibilities as directed.

Qualifications
Education:
B.S. degree from an accredited/approved school of Physical Therapy. Registered P.T. in the state of Minnesota by the Minnesota State Board of Physical Therapy.

Experience:
One to two years of Physical Therapy experience preferred. Special Knowledge,

Skills and Abilities:
Strong understanding of Physical Therapy principles and practices; Effective written and oral communication skills; Effective use of computer and other technology; Strong time management skills; Strong interpersonal skills; Reliable transportation to and from patient homes; Ability to lift and transfer clients using appropriate equipment; Knowledge regarding Medicare/Medicaid and third-party payors; Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all age groups (pediatric through geriatric). Must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patient's needs in relation to age and to provide the care needed as described in the department's policies and procedures. Primary Function & Major Responsibilities Evaluation and treatment of clients using physical therapy procedures to derive maximum potential benefit. Plans and implements treatment programs to ensure effective care of patients. Evaluates patient with the appropriate testing procedures to progress patient as ongoing evaluation indicates. Directs aides/assistants as required to assure quality patient care. Completes accurate and timely documentation and record keeping as required. Instructs patient and family and participates in discharge planning. Consults with referring practitioner to ensure quality care and treatments are provided to patients. Involves patient and/or family in goal setting in order to facilitate their active participation in treatment. Remains current on evaluations, treatment techniques, procedures, and other related topics. May supervise physical therapy or physical therapist assistant students. Provides good customer service to patients, staff and visitors. Functions within departmental policies, beliefs, and philosophies. Utilizes routine safety and infection control measures to ensure safe delivery of patient treatments. Attends and participates in meetings relating to patient care, quality assurance, infection control, internal affairs, etc., as scheduled for all staff. Attendance per HealthEast policy. Dress code per HealthEast policy. Maintains confidentiality in all aspects of the job to ensure appropriate maintenance of all privacy related issues. Performs all other related duties as requested to meet departmental needs.

Qualifications

Graduate of an accredited physical therapy program.

Good written and oral communication skills; Ability to stand and/or walk 90% of 8 hour shift; Ability to bend, reach, and assist with transfers. Be able to lift 50 pounds; Good organizational skills; Good interpersonal skills; Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to pediatric, adolescent, adult and geriatric patients.
