& Major Responsibilities  Critical Care Intensivist (Internal Medicine). Opening for Critical Care Intensivist (Internal Medicine) at St. Joseph?s Hospital in St. Paul, MN. This position involves the following duties: Manage the care for patients in the St. Joseph?s Hospital Neuro ICU and directly evaluate all new admissions to the ICU. Provide all necessary critical care services to neurology patients at the Hospital who are in or headed to the ICU, resuscitating and supporting critically ill neurological patients, minimizing secondary neurological injury and facilitating patients? transition to recovery environment. Ensure referred and assigned patients are properly and efficiently cared for and receive appropriate and responsive medical care according to their condition, symptoms, and diagnosis. Provide consultations for neuro-critical care and post procedural ICU patients. Provide at least daily updates on the condition of each patient being managed by intensivist service. Complete effective verbal communication with patient?s care team throughout patient?s stay and at key times, such as admission, care conferences with families, change in patient condition, and upon discharge or transfer from the unit. Document patient diagnosis, treatment, orders and clinical progress properly and correctly in order to allow for proper billing. Diagnose and provide critical care and services to inpatients at St. Joseph?s Hospital. Provide consultations for requesting physicians and services on a timely basis. Lead daily multi-disciplinary care rounds and conduct family/patient care conferences as needed. Function as a spokesperson for Neuro-Intensivist program for St. Joseph?s Hospital in a variety of settings. Work with medical directors and hospital administration to advance and grow the HealthEast Brain Aneurysm Center, and other ICU services, and participate in clinical research as needed.

Assess and manage Intensive care patients with primary work focusing on the Neuro Intensive Care population. You will work closely with a full interdisciplinary team to ensure the delivery of evidence based, high quality, compassionate care to patients and their families. Participate in research as requested.

<br><br><b>Qualifications</b><br>  Qualified candidate must have MD, DO or foreign equivalent, license to practice medicine in the state of Minnesota, 36 months medical residency training in Internal Medicine, 24 month fellowship in Critical Care Medicine, and be board certified/eligible in critical care. MAJOR RESPONSIBILITIES:

1. Provides primary care and consultative services to patients assigned to the Hospital Medicine Service.

Standards:

a. Develops plan of care in conjunction interdisciplinary staff to ensure quality care of patients.

b. Provides patient and family members with proper information regarding care plan and conducts care conferences as appropriate.

c. Works closely with counselors and nurses to ensure care is delivered timely and appropriately according to each patient?s individual need.

3. Completes effective and timely verbal communication with the patient?s primary physician at various stages during the process, such as admission, care conferences with interdisciplinary team & families and upon discharge.

Standards:

a. Assists in obtaining pertinent information from primary care and referring physicians.

b. Proactively communicates with other clinicians in order to affect a coordinated and timely transfer of information of necessary information to improve the quality of care for patients.

c. Documents all significant communications in the patient?s chart or through other appropriate mechanisms.

d. Dictates discharge summaries to primary care physicians within 12-24 hours of discharge to allow for timely communication after discharge.

4. Functions as a member of the Clinical Care Team to achieve patient outcomes within established parameters, through the development, communication and the implementation of the plan of care.

Standards:
a. Utilizes all interdisciplinary care team members and their resources or expertise to develop and implement the plan of care.

b. Returns pages and phone calls in a timely fashion to clarify orders, arrange discharges, follow-up on tests, and answer patient and family questions.

c. Works closely with the Care Coordinator to ensure the details needed to achieve these outcomes is in place.

5. Documents patient diagnosis, treatment, orders and clinical progress properly and correctly in order to allow for proper and correct billing.

Standards:

a. Attends all required educational sessions related to patient documentation and reimbursement and demonstrates thorough knowledge of this area when documenting individual patient information.

b. Uses approved abbreviations and meets all required medical staff rules and regulations regarding documentation in the clinical record.

c. Documents H&P information to allow for complete collection of co-morbidity and complication information for use by hospital medical records and coding staff.

d. Times and dates all progress notes. Prints signature every time.

e. Meets with Coding staff regularly and at request to review coding issues. Works closely with them to ensure appropriate billing for each encounter.

f. Submits charges on a daily basis.

6. Functions as a spokesperson for Hospitalist program and for HealthEast in a variety of settings, both internal and external, to improve awareness of service line and benefits to patient, physicians, the community and the hospital.

Standards:

a. Demonstrates confidence and competency in the role of Behavioral Health Hospitalist to patients, staff members and others to enhance understanding of program and the benefits.

b. Describes aspects of the Hospitalist program to clinical staff and referring physicians as needed.

7. Provides care to patients in other settings as required to ensure quality and the continuum of patient care while ensuring patient safety and satisfaction.

Standards:

a. Sees patients in the outpatient setting for follow up after the completion of chemical dependency treatment.

b. Responds to request for chemical dependency evaluation needs within the hospital as requested.

c. Serves on related hospital committees to bring perspective of service into policies and procedures with emphasis on quality improvement, clinical advantage and operational effectiveness and efficiency.

8. Participates in Graduate Medical Education programs including serving as teaching service faculty as requested by the Hospital Medical Director.

Standards:

a. Identifies and assigns patients to rotating resident physicians designed to provide educational opportunities appropriate for level of training.

b. Meets on a regular basis with rotating residents and other assigned students to review and supervise patient care and meet all requirements in the educational agreements and Rules and Regulations.

c. Provides timely feedback and evaluations to faculty and medical staff leaders regarding resident and student progress and potential and actual quality of care concerns.

9. Works closely with the Operations Manager of Hospital Medicine or designee to ensure the Hospitalist Service meets and exceeds the needs of its customers.

a. Frequently communicates with the director regarding census, assignment of patients to the service and any customer service needs.

b. Cooperates in the development of a work/call schedule that secures 365 days/year coverage.

c. Collaborates with the Operations Manager to assist in the continual improvement of the service and actively participates in strategic planning for growth and development.

Education:<br>
- Valid MD License in the State of Minnesota. Board Certified or Board Eligible in Family Medicine by the American Board of Family Medicine or Internal Medicine by the American Board of Internal Medicine. Addiction Medicine trained preferred. Active DEA registration certificate including buprenorphine waiver.

<br><br>Experience:<br>
- Demonstrated competence in practicing medicine. Hospital based inpatient management experience preferred. Experience working with patients who have chemical dependency and/or mental health diagnosis preferred. Previous experience working in a total quality improvement environment preferred

Our Pediatric department combines primary care, consultation and teaching in their practice. Through a teaching affiliation with the University of Minnesota, the staff keep up to date on new developments in the field of pediatric medicine and share this knowledge with other physicians and medical residents. Each facet of the practice centers on caring for the health of children in the community with special emphasis on wellness and prevention.

Examines adult patients to ensure proper injury care and disease prevention, diagnosis, treatment, and recovery. Requests necessary tests, and follow-up visits, and refers patients to specialists, as necessary. Relies on experience and judgment to plan and accomplish goals. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. Requests necessary tests and follow-up visits and refers patients to specialists, as necessary. Ensures clinical excellence at respective sites and facilities through quality patient experience, operational excellence and clinical excellence.

Physically present at designated clinic(s) and/or hospital(s) to see, diagnose, and treat patients per schedule established by the Site Medical Director or designee.

Conducts or arranges for patient rounds at hospitals or other care facilities at which Physician?s patients have been admitted or are in residence.

Provides on call services in accordance with requirements of the Corporation and in accordance with the schedule established by the Site Medical Director or designee.

Participate in extra service to provide ?coverage? for colleagues during their Continuing Medical Education time or vacations.

Plans and carries out medical care program for adults to aid in mental and physical growth and development.

Attends and participates in the designated monthly business meeting(s).

Attends and participates in special meetings as called for by the HMRI leadership, the site ?clinic? manager, or the Site Medical Director.

Attends all HMRI membership meetings.

Participates in orientation program designed to provide information regarding coding, billing, and documentation procedures used by the corporation, including compliance with all appropriate laws, rules, and regulations regarding Medicare, Medicaid, and any other third party payer, as well as attending any required ongoing training regarding such policies and procedures.

Attends Continuing Medical Education (CME) meetings to provide credit hours adequate to maintain state license and specialty society membership. All Continuing Medical Education meetings or conferences for which HealthEast (HMRI) will have a financial obligation must be approved by the Site Medical Director (content) and the site Clinic Manager (time absent from work to insure adequate number of providers).

Meets regularly with Clinic Manager to establish clinic goals, address clinic issues, and formulate and implement strategies on a weekly or bi-weekly basis.

Maintains status as a provider under and complying with the rules and regulations of the Medicare and Medicare programs, workers? compensation, managed care and direct contracting arrangements, and any other programs the corporation participates.

Participates fully in any managed care contracts or similar agreements entered into by the corporation.

Provides timely, accurate, and complete documentation of medical services in the medical record, both in clinic and in the hospital.

Appropriately codes and provides timely completion of all charge tickets representing the professional services rendered to patients, the appropriate level of documentation of such services noted in the patient?s medical record to support such coding, and physician?s full compliance with all approved policies and procedures related to such processes
Continually examines opportunities for standardization and/or consolidation within Clinic and HealthEast.

Understands and supports technology needs of clinic in association with corporate information technology department. Complies with standards and requirements of all accrediting, licensing and governmental agencies pertaining to the area of operational responsibility.

Identifies and recommends changes that will improve service, productivity, quality and/or financial performance.

Responsible for planning using appropriate planning and decision making tools.

Supports overall clinic initiatives.

Provide management for department that ensures proper utilization of human resources.

<br><br><b>Qualifications</b><br>  Education:<br>
Valid MD License in the State of Minnesota. Board Certified or Board Eligible in Internal Medicine.

The medical staff includes more than 170 physicians as well as numerous certified nurse midwives. Our adult, pediatric and geriatric nurse practitioners round out a compassionate and experienced team of nurses and other support staff. Our clinics span the St. Paul area with 14 different sites and focus on providing prompt, personalized patient care with same day access for acute services. Our network size and diversity allows us to offer a full range of services for patients of all ages, including family practice, pediatrics and internal medicine.

<br><br><b>Primary Function</b> 
& Major Responsibilities  Responsible for providing care to patients and residents at designated sites and facilities as assigned. Examines and treats members of family, regardless of age or sex. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. Requests necessary tests and follow-up visits and refers patients to specialists, as necessary. Ensures clinical excellence at respective sites and facilities through quality patient experience, operational excellence and clinical excellence.

Physically present at designated clinic(s) and/or hospital(s) to see, diagnose, and treat patients per schedule established by the Site Medical Director or designee.

Conducts or arranges for patients patient rounds at hospitals or other care facilities at which Physician?s patients have been admitted or are in residence.

Provides on call services in accordance with requirements of the Corporation and in accordance with the schedule established by the Site Medical Director or designee.

Participate in extra service to provide ?coverage? for colleagues during their Continuing Medical Education time or vacations.

Plans and carries out medical care program for children from birth through adolescence to aid in mental and physical growth and development.

Attends and participates in the designated monthly business meeting(s).

Attends and participates in special meetings as called for by the HMRI leadership, the site ?clinic? manager, or the Site Medical Director.

Attends all HMRI membership meetings.

Participates in orientation program designed to provide information regarding coding, billing, and documentation procedures used by the corporation, including compliance with all appropriate laws, rules, and regulations regarding Medicare, Medicaid, and any other third party payer, as well as attending any required ongoing training regarding such policies and procedures.

Attends Continuing Medical Education (CME) meetings to provide credit hours adequate to maintain state license and specialty society membership. All Continuing Medical Education meetings or conferences for which HealthEast (HMRI) will have a financial obligation must be approved by the Site Medical Director (content) and the site Clinic Manager (time absent from work to insure adequate number of providers).

Meets regularly with Clinic Manager to establish clinic goals, address clinic issues, and formulate and implement strategies on a weekly or bi-weekly basis.

Maintains status as a provider under and complying with the rules and regulations of the Medicare and Medicare programs, workers? compensation, managed care and direct contracting arrangements, and any other programs the corporation participates.

Participates fully in any managed care contracts or similar agreements entered into by the corporation.

Provides timely, accurate, and complete documentation of medical services in the medical record, both in clinic and in the hospital.

Appropriately codes and provides timely completion of all charge tickets representing the professional services rendered to patients, the appropriate level of documentation of such services noted in the patient?s medical record to support such coding, and physician?s full compliance with all approved policies and procedures related to such processes.

Continually examines opportunities for standardization and/or consolidation within Clinic and HealthEast.

Understands and supports technology needs of clinic in association with corporate information technology department. Complies with standards and requirements of all accrediting, licensing and governmental agencies pertaining to the area of operational responsibility.

Identifies and recommends changes that will improve service, productivity, quality and/or financial performance.

Responsible for planning using appropriate planning and decision making tools.

Supports overall clinic initiatives.

Provide management for department that ensures proper utilization of human resources.

Provides fiscal management for dept/service/team to ensure proper utilization of financial resources

Maintains confidentiality in all aspects of the job.

Performs other related duties as assigned.
<br><br><b>Qualifications</b><br>  Education:<br>
Valid MD or DO License in the State of Minnesota. Board Certified or Board Eligible in Family Medicine.

<br><br>Experience:<br>
Demonstrated competence in practicing medicine. Minimum of three years practicing medicine preferred.

<br><br>Special Knowledge, Skills, and Abilities:<br>
Demonstrated clinical expertise;
Demonstrated organizational, problem solving, and conflict resolution skills;
Current clinical skills regarding management of medical care and use of equipment;
BLS and ACLS;
Excellent interpersonal skills to work successfully with all disciplines including medical and support staff;
Clear understanding of Care Management and Performance Improvement approaches to clinical practice;
Knowledge of local healthcare market;
Previous experience with developing and maintaining effective medical staff relationships;
Effective liaison with HealthEast Medical Staff;
Extensive PC knowledge and skills;
Excellent verbal and written communication skills;
Demonstrated ability to work independently;
Ability to travel to multiple work sites/facilities;
Knowledge of coding/reimbursement issues;
Demonstrates the knowledge and skills necessary to provide care appropriate to patients of all age groups. Must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to gather data reflective of the patient?s status, identify patient needs in relation to age and to provide the care needed as described in the department?s policies and procedures and this job description.;
Ability to stand, walk, or sit for an extended period of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than whole hand(s) or arm(s). Communication skills using the spoken word. Ability to see within normal parameters. Ability to hear within normal range. Ability to stoop, bend, push or lift 50 pounds.
