What to expect

Our clinical team begins with a detailed history and review of medical records. Medical, neurological and psychiatric examinations include intellectual and functional status assessments. During this time, additional diagnostic studies may be offered.

Our multi-disciplinary team includes physicians, neuropsychologists, psychologists, nurses, social workers, physical, occupational, speech and recreational therapists. Our goal is to provide a full evaluation and complete plan through a unique combination of services that work together to provide effective and meaningful treatment for patients and their caregivers.

Education and support

Education and support are key for the best possible outcomes for both patients and families. Education is provided at every clinic visit and aims to empower our patients and their caregivers and to help in understanding and removing the stigma associated with dementia. We also offer both classes and support groups to complement services provided in our clinic.

Educational classes

Throughout the year, we offer educational classes for families and patients with cognitive and memory conditions. Classes are designed to provide up to date information to facilitate greater understanding of the illnesses we treat and also to inform patients and families regarding available community resources to assist in care. These classes help patients and caregivers gain a greater understanding about how to live better with a dementia diagnosis.

Outpatient Services referral form

Referrals are not required to make appointments at our Memory Loss, Brain Injury or Concussion clinics, or at the Capistrant Center. But be sure to check with your insurance plan for info on coverage for outpatient services.

Neuropsychology and therapy programs

A referral form completed by your referring physician is required for neuropsych, and for outpatient physical, occupational, speech and other therapy programs.
